Job Title: Mental Health Support Worker

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Mental Health Support Worker, you will play a vital role in supporting an individual with a diagnosis of Psychosis, providing 1:1 care and assistance in their own home.

About the Role:

The successful candidate will have experience in social care, ideally within mental health, and a good understanding of supporting individuals with complex health needs and Learning Disabilities. You will be responsible for implementing detailed care plans and risk assessments, providing encouragement and support to the individual, and promoting their independence.

Key Responsibilities:

  • Providing 1:1 support to an individual with a diagnosis of Psychosis
  • Implementing detailed care plans and risk assessments
  • Providing encouragement and support to the individual
  • Promoting the individual's independence
  • Working collaboratively with the multi-disciplinary team to ensure the individual's needs are met

Requirements:

  • Experience in social care, ideally within mental health
  • Good understanding of supporting individuals with complex health needs and Learning Disabilities
  • Ability to implement detailed care plans and risk assessments
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
