Position Overview

An exceptional opportunity has emerged for a Band 4 Nurse Practitioner (Assistant Practitioner/Nursing Associate) to become a vital member of our Transitional Care team. This dedicated team includes midwives, nursery nurses, and healthcare support personnel, providing care for up to 10 infants alongside their mothers in a specialized Transitional Care unit.

This role is crucial in maintaining the bond between mothers and their infants, assisting resident mothers as primary caregivers for their babies who require care beyond standard newborn protocols. Transitional care serves to avert unnecessary admissions to the neonatal unit while offering enhanced support for small or preterm infants. As a successful candidate, you will deliver the highest quality of assessment, planning, implementation, and evaluation of tailored care for infants receiving treatment in Transitional Care.

Key Responsibilities

  • Foster a welcoming and supportive environment for parents and relatives, enabling them to learn effective caregiving techniques for their infants.
  • Employ diverse communication strategies to engage effectively with the infants' families, the multidisciplinary team, other departments within the organization, and external agencies.
  • Conduct necessary observations on infants as required.
  • Provide support for breastfeeding initiatives.
  • Coordinate with the neonatal team regarding the transfer of infants to and from the unit or Transitional Care ward.
  • Keep parents and caregivers informed about their infant's progress, ensuring sensitivity to their needs for courtesy, dignity, and privacy, while always maintaining a friendly atmosphere.
  • Act as an advocate for the infant, promoting effective communication and collaboration with all members of the multidisciplinary team.
  • Collaborate closely with the midwife under registered supervision.

Qualifications and Experience

Essential

  • Higher Apprenticeship in Health Level 5 or Foundation Degree in Health and Social Care Practice or Higher Professional Diploma in Health and Wellbeing or a recognized equivalent course.
  • Numeracy and Literacy Level 2 or equivalent.
  • Evidence of full-time educational achievements.
  • Basic IT skills, including word processing, email, and data collection.
