As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with complex needs.

Key Responsibilities
  • Develop and implement person-centered care plans in collaboration with the individual, their family, and other healthcare professionals.
  • Conduct comprehensive assessments and evaluations to identify individual needs and develop effective care strategies.
  • Provide direct care and support to individuals with learning disabilities, including medication management, wound care, and mobility assistance.
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions and effective communication.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Registered Nurse with a valid nursing license and experience working with adults with learning disabilities.
  • Strong communication and interpersonal skills, with the ability to build effective relationships with individuals, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
