Key Responsibilities:

  • Provide advocacy and support services to vulnerable adults with profound and multiple needs.
  • Support individuals to make informed decisions about their health and social care.
  • Work collaboratively with public and voluntary organizations to ensure seamless support services.
  • Develop and maintain strong relationships with clients, families, and professionals to ensure effective support.
  • Conduct thorough assessments to identify individual needs and develop personalized support plans.
  • Provide emotional support and guidance to individuals and their families.
  • Collaborate with healthcare professionals to ensure comprehensive care and support.
  • Stay up-to-date with relevant legislation and policies affecting vulnerable adults.
  • Participate in regular training and development opportunities to enhance skills and knowledge.

Requirements:

  • Previous experience in advocacy, social care, or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent problem-solving and decision-making skills.
  • Proficiency in computer literacy and ability to use software applications.
  • Valid driver's license and access to a vehicle.
