Position Overview

We are seeking a dedicated Rehabilitation Case Coordinator to join our dynamic team at Health Case Management Limited. This role is ideal for individuals with a background in sports therapy or rehabilitation who are looking to leverage their clinical expertise in a supportive office environment.

Role Responsibilities

  • Clinical Service Delivery: Conduct thorough and consistent telephone assessments to ensure optimal clinical outcomes.
  • Plan, implement, and evaluate tailored interventions and clinical pathways to facilitate patient recovery and case resolution.
  • Employ the bio-psychosocial model during patient interactions and clinical decision-making.
  • Utilize every patient contact as an opportunity to educate and provide clinical support.

Patient Care:

  • Collaborate with colleagues to share responsibility for patient cases, fostering a team-oriented environment.
  • Enhance service delivery by proactively supporting team initiatives.
  • Uphold the highest standards of patient care, ensuring accurate and clear clinical guidance.
  • Adopt a forward-thinking, patient-centered approach to navigate case management challenges.

Essential Qualifications:

  • A relevant clinical degree (sports therapy, sports rehabilitation, or physiotherapy).
  • A minimum of one year of clinical or hands-on postgraduate experience, or previous expertise in case management or vocational rehabilitation.
  • Current registration with the relevant professional body (e.g., BASRaT, SST, HCPC) or the ability to obtain membership swiftly.
  • Strong written and verbal communication skills, along with excellent attention to detail and time management abilities.

Desirable Qualifications:

  • Experience in delivering high-quality customer service or proven case management experience.
  • Relevant experience in health, legal, or insurance sectors.
  • Membership in CMS UK.
