About the Role

We are seeking a highly skilled and experienced Rehabilitation Case Manager to join our team at HCML. As a key member of our clinical team, you will play a vital role in delivering high-quality rehabilitation services to our clients.

Key Responsibilities
  • Clinical Service Delivery
    • Deliver comprehensive and clinically robust telephonic and video assessments to ensure the most effective outcomes for our clients.
    • Assess, plan, implement, and evaluate interventions and clinical pathways to expedite patient recovery and case closure.
    • Utilize the bio-psychosocial model to inform clinical decisions and ensure patient-centered care.
    • Capture patient outcomes to demonstrate value to our customers.
  • Patient Care
    • Shared responsibility for patient cases, including owning individual caseloads and supporting colleagues.
    • Support the optimization of service delivery by identifying areas for improvement and promoting a culture of continuous improvement.
    • Adopt a patient-centered approach to overcome case management hurdles and work collaboratively with the team to identify priorities.
  • Leadership and Operational Management
    • Demonstrate robust clinical knowledge and support team members in their roles.
    • Liaise professionally with clients, insurers, and other internal and external parties to provide an excellent service.
Requirements
  • Experience
    • 12 months of clinical/hands-on experience, with previous case management or vocational rehabilitation expertise.
  • Skills and Knowledge
    • Excellent written and verbal communication skills, with good attention to detail and time management skills.
    • Experienced use of Microsoft Office software, including Word, Excel, and PowerPoint.
  • Qualifications and Training
    • Relevant clinical degree (Sports Therapy, Sports Rehabilitation, and Physiotherapy).
    • Current professional registration(s) of your relevant professional body (e.g., BASRaT or SST, HCPC).
