Job Summary

We are seeking a highly skilled and dedicated individual to join our team as a Healthcare Services Coordinator at K & W Healthcare. The successful candidate will be responsible for providing case management and care planning to patients with long-term conditions, with a focus on those aged 65+.

Key Responsibilities
  • Support the Complex Patient Management Group (CPMG) team and General Practices in identifying patients for care planning and case management.
  • Liaise with healthcare providers, social care providers, and voluntary sector organizations to collate information for care planning and case management.
  • Provide administrative, communication, and performance management support to individual GP Practices and the CPMG team.
  • Facilitate effective delivery of patient-centered, personalized health and social care, monitoring progress and reporting outcomes.
  • Contribute to patient reviews and care planning within appropriate time frames.
  • Ensure personalized care plans remain up-to-date and are evaluated and revised as necessary.
Requirements
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Strong organizational and time management skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Knowledge of healthcare policies and procedures.
Working Conditions

The post holder will be based at Willesden Medical Centre and will be required to visit patients in their homes and at GP Practices across Kingsbury and Willesden. The successful candidate will also be required to attend meetings and training sessions as needed.
