Role Overview

We are in search of a driven Field Care Team Leader to become an integral part of our organization and contribute significantly to our goal of delivering exemplary care to our clients.

Key Responsibilities

As a Field Care Team Leader, you will collaborate closely with the Registered Manager and Head of Governance. Your primary responsibilities will include:

  • Acting as the primary liaison for staff and service users, providing monthly supervision to team members, assessing their competencies, and conducting regular visits to service users to ensure their satisfaction and well-being.
  • Overseeing and mentoring staff, ensuring effective onboarding and guidance for new lead carers in your designated area.
  • Assisting the Registered Manager in maintaining compliance with CQC Regulations.
  • Facilitating the seamless execution of care packages through client visits and engagement with external stakeholders.
  • Developing and updating care plans and risk assessments for individuals, while supporting the auditing of care placements.
  • Providing weekly feedback to the Registered Manager, Quality Manager, and Clinical Services Manager regarding the care provided to each individual.
  • Preparing reports to support successful placements.
  • Ensuring the delivery of high-quality care consistently.
  • Building and nurturing essential relationships with the individuals we support and their families.
Qualifications and Experience

The ideal candidate will possess:

  • A QCF Level 4 qualification in Health and Social Care or Level 3 with significant relevant experience.
  • Previous experience in community care is preferred.
  • Experience in both adult and children's services.
  • A valid UK driving license is advantageous.
