Care Home Practitioner

Four Seasons Health Care is seeking dedicated Care Home Assistant Practitioners (CHAPs) to join our team and make a real difference in the lives of our residents.
As a CHAP, you will work closely with Registered Nurses to ensure that every resident receives the highest quality person-centered care. Your role is vital in maintaining a supportive and nurturing environment where residents feel valued and respected.
Your Responsibilities:- Collaborate with Registered Nurses to deliver exceptional medical and personal care to each resident, consistently monitoring the standard of care provided.
- Play a key role in ensuring residents' well-being by actively observing their needs and identifying any changes that may require adjustments to their care plans.
- Support the Home Manager in implementing policies, procedures, and business objectives, contributing to the smooth operation of the home.
- Identify training needs within the team and assist in developing and delivering effective training programs to enhance staff skills and knowledge.
- Conduct regular staff supervisions and appraisals, ensuring they meet external regulatory guidelines and contribute to a culture of continuous improvement.
- Serve as a primary point of contact for residents, their families, and healthcare professionals, fostering open communication and building strong relationships.
- A genuine passion for providing high-quality care to older adults in a residential setting.
- Proven experience in a relevant care role, demonstrating full competence and a commitment to delivering exceptional resident care.
- Possession of a Level 3 Health and Social Care qualification or actively working towards it.
- A dedication to ongoing professional development and staying up-to-date with best practices in the care sector.
- Excellent communication, interpersonal, and organizational skills.
