About the Role

We are seeking a highly skilled and experienced Supported Living Manager to join our team in Powys. As a key member of our management team, you will be responsible for leading a team of support workers and ensuring that individuals with complex needs receive high-quality care and support.

The successful candidate will have a strong background in care management, with experience of working with individuals with mental health and communication needs, as well as a learning disability. You will be able to demonstrate a commitment to the 'Active Support Model' and have a proven track record of developing and implementing person-centred support plans.

Key Responsibilities
  • Lead a team of support workers to deliver high-quality care and support to individuals with complex needs
  • Develop and implement person-centred support plans, working in partnership with individuals, families, and other agencies
  • Ensure consistency in the implementation of support plans, health plans, and behavioural support plans
  • Work collaboratively with other agencies, parents, and support workers to achieve positive outcomes
  • Develop and maintain relationships with external partners, including local authorities and other service providers
  • Contribute to the development of the organisation's care and support services, including the development of new initiatives and projects
Requirements
  • Minimum QCF Level 3 or equivalent in a relevant field
  • Experience of working with individuals with mental health and communication needs, as well as a learning disability
  • Proven track record of developing and implementing person-centred support plans
  • Strong leadership and management skills, with the ability to motivate and inspire a team
  • Excellent communication and interpersonal skills, with the ability to build relationships with individuals, families, and other agencies
  • Ability to work collaboratively as part of a team, with a flexible and adaptable approach
  • Commitment to ongoing personal and professional development
