Your primary responsibility will be to effectively manage the coordination of care services, ensuring that our care workers are matched to clients based on their specific needs and preferences.
Key Responsibilities
- Develop and implement efficient care coordination processes to ensure timely and effective delivery of care services.
- Collaborate with care workers, clients, and other stakeholders to ensure seamless communication and coordination of care services.
- Conduct regular reviews of care plans to ensure they are up-to-date and meet the evolving needs of our clients.
- Identify opportunities for process improvements and implement changes to enhance the quality and efficiency of our care services.
- Maintain accurate and detailed records of client care plans, care worker schedules, and other relevant information.
- Provide exceptional customer service to clients, care workers, and other stakeholders, responding promptly to their needs and concerns.
- Work closely with our care management team to ensure that our care services are delivered in accordance with our company's policies and procedures.
- Contribute to the development and implementation of new care services and programs to meet the evolving needs of our clients.
Requirements
- Relevant qualifications in care management or a related field.
- Proven experience in care coordination or a related role, preferably in a home care setting.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, care workers, and other stakeholders.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
- Ability to work independently and as part of a team, with a flexible approach to working hours and responsibilities.
