Learning Disability Nurse Enhanced Health in Care Homes Specialist

We are seeking a highly skilled Learning Disability Nurse to join our Enhanced Health in Care Homes (EHCH) team at Southport and Formby Health (GP Federation). As a key member of our team, you will play a vital role in promoting the health and wellbeing of individuals with learning disabilities living in care homes.
Key Responsibilities- Work collaboratively with care home staff to develop and implement individualized care plans that meet the unique needs of residents.
- Provide clinical leadership and support to care home teams to ensure high-quality care and promote person-centered approaches.
- Develop and maintain relationships with local healthcare providers, social services, and community teams to ensure seamless care coordination.
- Conduct holistic assessments and provide expert advice on health and wellbeing strategies for individuals with learning disabilities.
- Work closely with the Primary Care Network (PCN) to identify gaps in local service provision and develop initiatives to reduce health inequalities.
- Current qualification and registration as a Learning Disability Nurse with the NMC.
- Experience of working with individuals with learning disabilities in community and healthcare settings.
- Excellent interpersonal and communication skills, with the ability to work effectively with stakeholders at all levels.
- Strong IT skills and ability to use digital technology to support care planning and record-keeping.
