Learning Disability Nurse

We are seeking a highly skilled Learning Disability Nurse to join our team at Southport and Formby Health (GP Federation) as an Enhanced Health in Care Homes Specialist. This is an exciting opportunity to work with our dynamic team to provide high-quality, integrated care to individuals with learning disabilities living in care homes.
Key Responsibilities- Work collaboratively with care home staff to develop and implement individualized care plans that meet the unique needs of residents with learning disabilities.
- Provide clinical support and guidance to care home teams to ensure that residents receive high-quality, person-centered care.
- Conduct holistic assessments and develop proactive care plans to support residents with learning disabilities in achieving their health goals.
- Work closely with the Primary Care Network (PCN) to identify gaps in local service provision and develop strategies to reduce health inequalities within the learning disability population.
- Develop and deliver education sessions to care home staff and other stakeholders on health education, screening processes, and conditions affecting individuals with learning disabilities.
- Embed the core NMC values and expectations of care, compassion, courage, communication, commitment, and competence into everyday practice.
- Current qualification/nurse registration with NMC
- Demonstration of CPD and revalidation
- Excellent interpersonal and communication skills
- Highly developed interpersonal skills, negotiation, conflict management, feedback, and partnership working
- Strong IT skills
- Ability to support people who have a learning disability on an individual and/or group basis
- Knowledge of up-to-date learning disability policy
- Knowledge of EMIS clinical software
