Learning Disability Nurse Enhanced Health in Care Homes Specialist

We are seeking a highly skilled Learning Disability Nurse to join our Enhanced Health in Care Homes (EHCH) team at Southport and Formby Health (GP Federation). As a key member of our team, you will play a vital role in promoting the health and wellbeing of individuals with learning disabilities living in care homes.
Key Responsibilities- Work collaboratively with care home staff to develop and implement individualized care plans that meet the unique needs of residents.
- Provide clinical support and guidance to care home teams to ensure high-quality care provision.
- Conduct holistic assessments and develop proactive care plans to support residents with learning disabilities.
- Work closely with the Primary Care Network (PCN) to identify gaps in local service provision and develop strategies to reduce health inequalities.
- Develop and deliver education sessions to care home staff and other stakeholders on health education, screening processes, and conditions affecting individuals with learning disabilities.
- Current qualification and registration as a Learning Disability Nurse with the NMC.
- Experience of working with individuals with learning disabilities in community and/or healthcare settings.
- Strong interpersonal and communication skills, with the ability to motivate and inspire others.
- Ability to work collaboratively with stakeholders, including care home staff, PCN teams, and other healthcare professionals.
