Learning Disability Nurse

We are seeking a highly skilled Learning Disability Nurse to join our team at Southport and Formby Health (GP Federation) as an Enhanced Health in Care Homes Specialist. This is an exciting opportunity to work with our dynamic team and contribute to the development of high-quality, integrated patient services.
Key Responsibilities- Work collaboratively with Care Coordinators to coordinate and communicate individual care plans with system partners and develop relationships with care homes, community teams, mental health services, palliative care services, social services, and secondary care frailty teams.
- Follow standard operating procedures within the limits of your defined clinical competence, and this will be reviewed regularly as the role evolves.
- Work within your team and have the opportunity to escalate concerns and complexities to the multi-disciplinary team meetings.
- Identify and/or engage in locally organised shared learning opportunities as appropriate and as capacity allows.
- Support with the development and production of learning resources to improve patient experience within GP practices and care homes.
- Develop links with local support groups to ensure that patient voices are heard.
- Work with the Primary Care Network (PCN) to identify gaps within local service provision and work to reduce health inequalities within the learning disability provision.
- Support GP practices within the PCN to complete annual health checks for the learning disability population who do not reside within care homes.
- Work with the PCN on increasing the uptake of screening provision within the learning disability population, particularly care home residents, specifically around cancer priority areas such as bowel, cervical, and breast screening.
- Current qualification/nurse registration with NMC
- Demonstration of CPD and revalidation
- Ability to support people who have a learning disability on an individual and/or group basis
- Have a shared vision for better care for people with a learning disability
- Embed the core NMC values and expectations of care, compassion, courage, communication, commitment, and competence into everyday practice
- Ability to motivate others by influencing and inspiring others in new enhanced ways of working
- Ability to communicate with stakeholders and convey complex messages to different recipient groups, in particular those with learning disabilities
- Able to develop effective and mutually supportive relationships with key partners within and without organisations
- Able to organise and manage your own time and workload
- Ability to recover and recuperate quickly from difficult or challenging situations
- Knowledge of up to date learning disability policy
- Knowledge of EMIS clinical software
- Experience of recording care plans electronically
Southport and Formby Health (GP Federation) is a dynamic and expanding GP federation, working with Southport and Formby Primary Care Network (PCN) and other system providers to provide high-quality, integrated patient services. We are commissioned to provide the Enhanced Health in Care Homes Specification across Southport and Formby.
