About the Role:

We are seeking a dedicated and compassionate Support Practitioner to join our Helensburgh Day Responder service. As a key member of our team, you will play a vital role in providing effective and efficient support to individuals in their own homes, reducing the need for ambulance call-outs and hospital admissions.

Key Responsibilities:

  1. Provide high-quality support to individuals in their own homes, promoting independence and positive outcomes.
  2. Work collaboratively with colleagues to ensure a seamless service delivery.
  3. Contribute to the development and implementation of individual support plans.
  4. Participate in ongoing training and professional development to enhance skills and knowledge.
