About the Role

This is an exciting opportunity to make a positive impact in the lives of adults with complex needs, including learning disability and autism.

As a Lead Practitioner, you will provide individualised support to adults in their own homes, living in the community, and support them with everyday opportunities that promote independence and choice.

You will work in partnership with health teams, social work teams, Community justice teams, Police and advocacy to enable people to access everyday life experiences, including education, employment/volunteering, and social activities.

Key responsibilities include:

  • Providing individualised support to adults with complex needs
  • Supporting individuals within the community and engaging with their hobbies and interests
  • Attending various community groups and accompanying service users to attend activities they are passionate about
  • Assisting with personal care and ensuring comfort and wellbeing
  • Celebrating achievements and milestones to create a rewarding and positive environment
About You

No previous working experience is needed; full training will be provided to you.

You will have a genuine commitment to use your skills to support people to live their best lives.

You will be supported by an incredibly approachable management team, allowing you to expand on your existing knowledge whilst developing new skills.
