About the Role

We are seeking a skilled and experienced General Practitioner to join our team as a Community GP Leader. This is an exciting opportunity to work in a dynamic and innovative environment, leading a clinical team to provide proactive and reactive general medical services to the housebound population.

Key Responsibilities
  • Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits where needed.
  • Home visiting for clinically complex cases, end of life care, diagnostic uncertainty, and complex prescribing decisions.
  • Senior clinical triage for the Acute Home Visiting service.
  • Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
  • Opportunity for involvement in service development as the team expands.
  • Involvement in MDT meetings.
  • Provide holistic comprehensive assessment and enhanced advance planning.
  • Maximise best care in the patient's own home to reduce the need for hospital or care home admissions.
  • Provide end of life care in line with the patient's wishes.
  • Quality improvement activity.
  • Opportunities for teaching and mentorship.

Person Specification
  • A vocationally trained and accredited GP.
  • Current registration with GMC.
  • On the GP performers list.

We are looking for a candidate with experience and evidence of an interest in care of the elderly, as well as excellent organisational and communication skills. The successful candidate will be able to work effectively as a member of a team, with a flexible and supportive approach. Experience of multidisciplinary working and senior clinical triage is desirable.
