We are seeking a highly skilled and experienced General Practitioner to lead our clinical team in the Home Visiting Service. As a key member of our team, you will be responsible for providing senior clinical leadership, developing and implementing clinical pathways, and working collaboratively with our multidisciplinary team to deliver high-quality patient care.
Key Responsibilities- Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits where needed.
- Home visiting for clinically complex cases, end of life care, diagnostic uncertainty, and complex prescribing decisions.
- Senior clinical triage for the Acute Home Visiting service.
- Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
- Opportunity for involvement in service development as the team expands.
- Involvement in MDT meetings.
- Provide holistic comprehensive assessment and enhanced advance planning.
- Maximise best care in the patient's own home to reduce the need for hospital or care home admissions.
- Provide end of life care in line with the patient's wishes.
- Quality improvement activity.
- Opportunities for teaching and mentorship.

Person Specification- A vocationally trained and accredited GP.
- Current registration with GMC.
- On the GP performers list.
- Experience and evidence of an interest in care of the elderly.
- Understanding of adult safeguarding and Deprivation of liberty procedures.
- Flexible, supportive, and collaborative.
- Recognise the benefits of multiagency and multidisciplinary team working.
- Ability to work as part of a multi-disciplinary team with vulnerable patients and their families, carers often at end of life or with complex care needs.
