About the Role

Belper PCN is seeking a skilled Community GP to lead our established Home Visiting Service. As a key member of our clinical team, you will provide senior clinical triage, clinical support, and mentorship to our multidisciplinary team.

Main Responsibilities

  • Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits where needed.
  • Home visiting for clinically complex cases, end of life care, diagnostic uncertainty, and complex prescribing decisions.
  • Senior clinical triage for the Acute Home Visiting service.
  • Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
  • Opportunity for involvement in service development as the team expands.
  • Involvement in MDT meetings.
  • Provide holistic comprehensive assessment and enhanced advance planning.
  • Maximise best care in the patient's own home to reduce the need for hospital or care home admissions.
  • Provide end of life care in line with the patient's wishes.
  • Quality improvement activity.
  • Opportunities for teaching and mentorship.

Person Specification

  • A vocationally trained and accredited GP.
  • Current registration with GMC.
  • On the GP performers list.

Desirable Qualifications and Skills

  • Experience and evidence of an interest in care of the Elderly.
  • Understanding of adult safeguarding and Deprivation of liberty procedures.

Personal Qualities and Skills

  • Flexible, supportive, and collaborative.
