About the Role

We are seeking a skilled and experienced General Practitioner to lead our Home Visiting Service as a Community GP. This is an exciting opportunity to work in a dynamic and innovative environment, collaborating with a multidisciplinary team to deliver high-quality care to our patients.

Key Responsibilities
  • Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits as needed.
  • Conduct home visits for clinically complex cases, end-of-life care, diagnostic uncertainty, and complex prescribing decisions.
  • Perform senior clinical triage for the Acute Home Visiting service.
  • Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
  • Contribute to service development as the team expands.
  • Participate in multidisciplinary team meetings.
  • Provide holistic comprehensive assessments and enhanced advance planning.
  • Maximize best care in the patient's own home to reduce hospital or care home admissions.
  • Deliver end-of-life care in line with the patient's wishes.
  • Engage in quality improvement activities.
  • Offer opportunities for teaching and mentorship.

Requirements
  • Vocationally trained and accredited GP.
  • Current registration with the GMC.
  • On the GP performers list.
  • Experience and evidence of interest in care of the elderly.
  • Understanding of adult safeguarding and deprivation of liberty procedures.
