We are seeking a skilled and experienced General Practitioner to lead our Home Visiting Service as a Community GP. This is an exciting opportunity to work in a dynamic and innovative environment, collaborating with a multidisciplinary team to deliver high-quality care to our patients.
Key Responsibilities- Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits as needed.
- Conduct home visits for clinically complex cases, end-of-life care, diagnostic uncertainty, and complex prescribing decisions.
- Perform senior clinical triage for the Acute Home Visiting service.
- Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
- Contribute to service development as the team expands.
- Participate in multidisciplinary team meetings.
- Provide holistic comprehensive assessments and enhanced advance planning.
- Maximize best care in the patient's own home to reduce hospital or care home admissions.
- Deliver end-of-life care in line with the patient's wishes.
- Engage in quality improvement activities.
- Offer opportunities for teaching and mentorship.
Belper PCN is an association of four GP practices working together to provide a range of health services to our combined list of 45,600 patients. We prioritize teamwork and innovation, striving to improve patient care and contribute to the sustainability and growth of primary care.
Requirements- Vocationally trained and accredited GP.
- Current registration with the GMC.
- On the GP performers list.
- Experience and evidence of interest in care of the elderly.
- Understanding of adult safeguarding and deprivation of liberty procedures.
- Flexible, supportive, and collaborative.
- Recognize the benefits of multiagency and multidisciplinary team working.
- Ability to work with vulnerable patients and their families, carers, often at end-of-life or with complex care needs.
- Ability to understand the competencies of others and support them to work within and at the top of those competencies.
- Excellent organizational and communication skills.
- Ability to work effectively as a member of a team.
- Experience of service improvement.
