Detailed Job Responsibilities

Medication Review and Optimization:

- Conduct face-to-face consultations to address medication-related concerns, including adverse effects, interactions, and queries from care facilities.

- Perform comprehensive medication reviews and manage repeat prescriptions for newly registered patients.

- Address medication inquiries via telephone, ensuring clarity on dosages and alternatives when necessary.

- Collaborate with other healthcare professionals to discuss complex medication regimens and compliance challenges.

- Assist in completing medication administration forms for nursing staff.

- Conduct medication reviews in nursing homes as required.

Medicines Reconciliation:

- Review requests for new medications from secondary care and communicate with relevant general practitioners regarding any discrepancies.

Medicines Information and Education:

- Keep colleagues informed about ongoing prescribing issues, new guidelines, and any changes in medication availability.

- Provide advice on cost-effective prescribing practices and budget considerations.

Prescribing Systems and Policies:

- Identify patients requiring medication reviews and collaborate with administrative staff to develop an effective invitation system.

- Work closely with the practice management team to enhance and implement medication management systems.

- Participate in audits related to prescribing targets and quality improvement initiatives.

Liaison with Community and Hospital Pharmacies:

- Support the dispensary team and utilize community pharmacy services for medication reviews.

Teaching and Training:

- Engage in the education of trainees and medical students, contributing to their learning experience within the practice.

Qualifications

Essential:

  • GPhC Registered Pharmacist

Desirable:

  • GPhC Independent Prescriber Qualification

Experience

Essential:

  • Minimum of two years of pharmacy experience, not limited to general practice.

Desirable:

  • Experience working within a general practice setting.
