About us
Cwmbran Village Surgery situated in Cwmbran, Torfaen, Wales - nestled on the Monmouthshire-Brecon Canal, just North of the city of Newport. The practice is an easy drive from Cardiff (30mins) and North Bristol (35mins), there are also excellent rail links with regular services to Newport, Cardiff and to the North.
The surgery benefits from ample parking and EV charging on site. We are a large team of clinical and non-clinical staff serving 16000 patients who are looking for the right person to join our team.
With a wealth of experience, we hope to be able to offer a supportive environment for the right candidate to thrive and develop their career within general practice. As a member of the re-training network, with two GP appraisers in-house and 3 RCGP trainers we are well equipped to support newly qualified staff looking for a supportive practice.
The practice is excellently organised across two sites by our experienced management team supported by a friendly and hard-working administrative team. With dedicated secretarial and prescribing roles within the practice that help facilitate an enjoyable working environment.

Job responsibilities
Medication Review andOptimisation :
Face to face clinics:
oTo discuss medication issues with patients asneeded and appropriate adverse effects/interactions, overdose/inadvertentingestion, OTC remedies, queries from care homes.
oTo carry out medication reviews and to discussspecific issues
oTo review and continue repeat medications fornew patients who have just registered with the practice, and suggest GPmedication review where appropriate.
To discuss medication issues withpatients as needed and appropriate by telephone, adverseeffects/interactions, overdose/inadvertent ingestion, OTC remedies, queriesfrom care homes.
To respond to queries to include,re-authorise repeats where there are queries, clarify doses, clarify products,give appropriate alternatives when availability issues.
To discuss specific patient andprescribing issues ( complicated regimes/polypharmacy, compliancedifficulties, multiple adverse effects, medication reduction regimes) withother clinicians as necessary.
To support the completion (medicationadministration) forms for district nurses for GPs to sign.
To visit nursing homes to carry outreviews of MAR sheets/patients medications when required.
Medicines Reconciliation:
To review secondary care requests for newmedication (as communicated by discharge summaries, outpatient letters, etc.)and raise any queries with relevant GP, and discuss how to respond wheninappropriate prescribing requests arise.
Medicines Information/Education:
To monitor and inform colleagues asrelevant about ongoing prescribing issues, new guidelines (national andlocal), new products being asked for by secondary care, manufacturing andsupply problems, new prescribing restrictions or contraindications, andindividual and systematic errors made by colleagues.
To advise on cost effective prescribingand prescribing budget issues.
To keep the prescribing folder on thepractice intranet up to date, with admin support.
Prescribing Systems and Policies:
To identify patients in need ofmedication review and develop the system of patient invitation with the adminteam.
To liaise with Lead GP, Practice Manger,admin and clinical staff in discussing, developing, and implementing medicinesmanagement systems in the practice.
To work with the GPs (especially theprescribing lead), practice manager & Prescribing Team to review, developand implement prescribing policies and strategies for the whole practice.
To suggest, design & carry out auditsin relation to prescribing targets, implementation of locality policies, QualityImprovement Framework and CEPP work.
To work with the GPs, practice manager,and dispensary manager on choosing and delivering targets for the localPrescribing Incentive Scheme (PIS).
To monitor the practices prescribingperformance (via the CCG prescribing dashboard, progress on the PIS, ePACT,etc.), and suggest changes to practice prescribing as appropriate.
To support the delivery of electronicprescribing.
To work with the dispensary team and leaddispensing GP to maximize profitability and to perform audits.
Liaison with community and hospitalpharmacies:
To support and advise dispensary whenneeded.
To make best use of community pharmacymedicine use reviews (MURs) for medication reviews.
To rationalise requests for dossettemedications. Dispensary will still be responsible for ringing round reavailability of products, taking initial calls, requesting new dosettes etc.
Teaching/training:
We are a training practice, and often havelearners (GP trainees, medical students and PN trainees) sit in onconsultations and occasionally having tutorials one-to-one. The PracticePharmacist will be sometimes asked to participate in the education of traineesin these settings
