Job Summary

The Practice Pharmacist is a key member of the clinical team at Cwmbran Village Surgery, utilizing their pharmacy expertise to support GPs and the practice in effective medicines management and optimal patient care.

Main Responsibilities
  • Medication Reviews and Optimisation
    • Conduct face-to-face clinics to discuss medication issues with patients, including adverse effects, interactions, and overdose prevention.
    • Review and continue repeat medications for new patients, suggesting GP medication review when necessary.
    • Discuss medication issues with patients by telephone and respond to queries, including re-authorizing repeats and clarifying doses.
    • Support the completion of medication administration forms for district nurses and visit nursing homes to review MAR sheets and patient medications.
  • Medicines Reconciliation
    • Review secondary care requests for new medication and raise queries with relevant GPs.
    • Discuss how to respond to inappropriate prescribing requests.
  • Medicines Information and Education
    • Monitor and inform colleagues about ongoing prescribing issues, new guidelines, and product availability.
    • Advise on cost-effective prescribing and budget issues.
    • Keep the prescribing folder on the practice intranet up to date.
  • Prescribing Systems and Policies
    • Identify patients in need of medication review and develop a system for patient invitation.
    • Liaise with the Lead GP, Practice Manager, and clinical staff to discuss, develop, and implement medicines management systems.
    • Work with GPs and the Prescribing Team to review and implement prescribing policies and strategies.
    • Monitor the practice's prescribing performance and suggest changes as necessary.
  • Liaison with Community and Hospital Pharmacies
    • Support and advise the dispensary team as needed.
    • Rationalize requests for dosette medications.
  • Teaching and Training
    • Participate in the education of trainees, including GP trainees, medical students, and PN trainees.
Person Specification
  • Qualifications
    • Essential: GPhC Registered Pharmacist.
    • Desirable: GPhC Independent Prescriber Qualification.
  • Experience
    • Essential: Minimum two years of pharmacy experience, not necessarily in General Practice.
    • Desirable: Experience of working in a GP practice.
