About the Role

We are seeking a highly motivated and experienced Senior Community Staff Nurse to join our dedicated Community Nursing team at North Cumbria Integrated Care NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality, patient-centred care in the community.

Key Responsibilities
  • Deliver holistic, patient-centred care in the community, including complex caseload management.
  • Liaise and work with primary health care teams and other Health and Social Care professionals to provide comprehensive care.
  • Support the Team Lead in providing clinical leadership and operational management of the community nurse team.
  • Manage the clinical caseload for the team, ensuring effective care coordination and patient outcomes.
Requirements
  • Registered Nurse with a strong background in community nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centred care.
  • Experience in complex caseload management and patient care coordination.
  • Strong leadership and management skills, with the ability to support and develop team members.
