About the Role

We are seeking a highly motivated and experienced Senior Community Staff Nurse to join our dedicated Community Nursing team at North Cumbria Integrated Care NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care in the community.

Key Responsibilities
  • Deliver holistic, patient-centered care in the community, including complex caseload management.
  • Liaise and work with primary health care teams and other Health and Social Care professionals to provide comprehensive care.
  • Support the Team Lead in providing clinical leadership and operational management of the community nurse team.
  • Manage the clinical caseload for the team, ensuring effective care delivery and patient outcomes.
Requirements
  • Registered Nurse with a strong background in community nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Ability to work in a changing environment, with a focus on patient-centered care.
  • Desirable: Community experience and knowledge of local health and social care services.
