We are seeking a skilled and compassionate Wound Care Nurse to join our team at Derbyshire Community Health Services NHSFT. As a Wound Care Nurse, you will play a vital role in providing high-quality care to patients with complex wounds, working closely with our multidisciplinary team to deliver evidence-based wound care.

Main Responsibilities
  • Assess and manage patients with a wide range of wounds, including diabetic foot ulcers, pressure sores, and venous leg ulcers.
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare professionals to achieve optimal wound healing outcomes.
  • Provide education and support to patients and their families on wound care and management, promoting self-care and independence.
  • Liaise with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care and coordination.
  • Participate in quality improvement initiatives and contribute to the development of wound care policies and guidelines.

Requirements

  • Ability to work independently and as part of a team, with a flexible approach to working practice.
