We are seeking a highly skilled and compassionate Registered Nurse to join our team at St Elizabeth Hospice. As a Nurse Specialist, you will play a vital role in providing holistic care and support to young adults living with progressive illness and complex health care needs.
Key Responsibilities- Assess and plan care for young adults with complex needs, ensuring their health and care requirements are met.
- Lead the short break care of up to three young adults, supervising care assistants and delegating tasks as needed.
- Support young adults to achieve their goals and plan weekend activities, promoting independence and self-management skills.
- Collaborate with the multi-disciplinary team to ensure seamless care delivery and effective communication.
- First-level Registered Nurse with current NMC registration.
- Minimum of 1 year post-qualification experience in palliative care or a related field.
- Excellent communication and organisational skills, with the ability to deal with complex matters.
- Ability to supervise the work of others and work within a multi-disciplinary team.
