An exceptional opportunity has emerged for a qualified and experienced registered nurse, social worker, or allied health professional to join the NHS West Yorkshire Integrated Care Board's Continuing Healthcare (CHC) Team as a Lead Coordinator within the Adult Physical Disability Division. The successful candidate will spearhead the development and oversight of a cohesive strategy for NHS CHC and Funded Nursing Care (FNC) within either a residential care facility or community environment. This includes the evaluation and reassessment of individuals who may qualify for, or are currently receiving, NHS CHC or FNC.
The Lead Coordinator will oversee and manage both clinical and non-clinical personnel, ensuring the effective administration and coordination of assessments, while also guaranteeing the formulation and continuous evaluation of specialized care packages where the NHS West Yorkshire Integrated Care Board serves as the primary commissioner or funding body.
Key Responsibilities

- Supervise a team of healthcare professionals in delivering CHC services for the Physical Disability and General Adult patient demographic.
- Ensure the verification, quality assurance, and auditing of CHC assessments to maintain a uniform approach to decision-making.
- Establish and manage a robust case management framework for individuals eligible for CHC.
- Provide clinical oversight and professional development opportunities for CHC Lead Nurses.
- Foster and maintain effective working relationships with key stakeholders, including local authorities, in managing jointly funded cases.
- Administer a designated budget, implementing a thorough review system to ensure care packages remain suitable and effective.

- Address complaints sensitively and promptly in accordance with organizational policies, striving for local resolution when feasible, and escalating to the Senior Clinical Operational Manager as necessary.
- Effectively communicate sensitive and complex information to individuals or professional forums, including NHS England and Health Ombudsman levels.

- Participate in or chair complex care planning meetings and reviews, employing effective negotiation techniques.
- Act as an independent decision-maker within agreed parameters concerning care package formulation and resource allocation.

- Produce timely advanced reports, papers, and analyses requiring prolonged concentration.
- Offer timely advice, information, or support to internal and external stakeholders regarding complex issues, adhering to policies, legislation, governance requirements, and best practices.
- Work autonomously on a daily basis, reporting directly to the operations manager.
- Act as an expert resource, providing professional advice on complex matters to patients, caregivers, healthcare facilities, general practitioners, social services, and the Health and Care Partnership regarding NHS Funded Healthcare.
- Contribute to safeguarding vulnerable adults and children in accordance with established procedures.
- Assist in reviewing and developing existing information management systems, contributing to an integrated approach to service or initiative management.
- Develop policies and procedures within the role that impact the broader organization as required.
- Ensure compliance with National Framework guidelines and all relevant Health and Care Partnership policies and procedures.
- Implement and adhere to relevant policies concerning safeguarding vulnerable adults, reporting as per local authority requirements.
- Provide regular reports on resource targeting and monitor implementation from a value-for-money perspective.
- Oversee and monitor all aspects of team budgets.
- Evaluate projects/functions within the designated portfolio against financial recovery/savings plans, providing high-quality project analysis.
- Identify opportunities for financial recovery while ensuring client needs are met and maintaining high-quality care standards without compromising safety and well-being.
- Collaborate with colleagues to ensure public finances are allocated appropriately, following procurement policies and procedures.
- Review the appropriateness of funds allocated to individual and group cases on an agreed basis.
- Work with the Operational Lead and Head of Service to determine optimal allocation of financial and physical resources to support care delivery and core service functions.
- Lead the coordination of training, development, and recruitment activities across the team.
- Manage staff, conducting appraisals, recruitment, and addressing any disciplinary or capability issues as necessary.
- Monitor staff attendance and ensure effective performance management regarding sickness.
- Follow the sickness policy and conduct meetings when triggers are met.
- Manage staff caseloads effectively.
- Contribute to the development of the service business plan, QIPP projects, and lead associated projects as required.
- Ensure compliance within the CHC team regarding the maintenance of the individual patient database, ensuring all information is accurate and relevant.
