Job Description:
Key Responsibilities- Undertake Continuing Healthcare (CHC) assessments and reviews for patients across all aspects of care, requiring a high level of assessment skills, tact, diplomacy, and effective liaison with colleagues, healthcare professionals, service users, carers, and family members.
- Complete Mental Capacity Act (MCA) assessments and Best Interest decisions where appropriate, working within the Deprivation of Liberty Safeguards (DoLS) framework, identifying and responding to the need for DoLS authorisation, which may involve working with legal professionals and the Court of Protection.
- Offer continuing support and advice to family and carers in complex and sensitive situations, requiring high interpersonal and reassurance skills, supporting the local resolution process in the case of complaints, appeals, and retrospective cases, and providing information on a timely basis.
- Develop and maintain effective communication networks with health and social care professionals, acting as a resource to them, and working with relevant health professionals, CHC colleagues, business support staff, senior leads, and other support staff within the CHC team to ensure a seamless and efficient service delivery.
- Exercise professional judgement when analysing and communicating complex and sensitive information on behalf of the CHC service, applying the associated criteria.
- Interpret national and regional guidance relating to the implementation of the CHC National Framework and subsequent developments within the CHC service, making decisions with regards to multi-disciplinary team CHC assessments to determine eligibility.
- Support Community Matrons/District Nurses in their case management roles for complex cases that are eligible for continuing healthcare, deputising for the CHC Team Leader when required.
- Deliver against organisational objectives, achieving quality outcomes, prioritising own workload, and working to tight deadlines, planning, organising, and implementing reviews and assessments, coordinating with Local Authority, family, and providers to ensure a holistic care plan is in place, including the production and co-production of care plans with Health and Care partners.
- Ensure personal health budgets are actively promoted with service users and their representatives, working closely to encourage a seamless delivery, identifying risk of abuse and need for protection from harm to self or others, and reporting in line with local safeguarding routes and governance arrangements.
- Ensure timely 'End of life' provision and reviews, following national guidance for clients fast-tracked through the CHC system, providing specialist nursing advice to secondary care, primary care, nursing homes, etc., to facilitate the continuing healthcare process, ensuring policies are implemented and adhered to, identifying and proposing changes for improvement to practices and procedures in own area, and implementing changes to policies and procedures.
- Approve a package of care in line with Place-based Partnership standing financial instructions and agreed thresholds, continually striving for delivering project/function outcomes, value for money, and greater efficiency, agreeing eligibility and making financial decisions within the Place-based Partnership standing financial instructions, and delivering formal and non-formal education programmes to Community and hospital staff, including nursing/residential homes, GPs, community nursing, and therapy staff, relating to the assessment and management of CHC patient groups in line with the latest research, evidence, and Department of Health/NICE guidance.
