Clinical Lead Nurse

We are seeking an experienced and skilled Clinical Lead Nurse to join our team in the Discharge Lounge, Integrated Hub at Huddersfield Royal Infirmary. The successful candidate will be responsible for coordinating discharge planning, liaising with care providers, and ensuring the smooth transition of patients from the ward to the discharge lounge.
**Key Responsibilities**
* Coordinating discharge planning and ensuring timely transfers of patients
* Liaising with care providers, care facilities, and family members to ensure effective discharge
* Recognising the need for further community referrals and creating an effective discharge plan
* Completing patient education and documentation on the electronic patient record
**Requirements**
* NMC Registered Nurse in a relevant field of practice
* Practice Assessor/Practice Supervisor qualification or willingness to complete
* Knowledge and skill to plan, deliver, and evaluate high-quality nursing care
* IT literacy and ability to use NHS systems
