About the Role

We are seeking a skilled and compassionate individual to join our team as a Care Facilitator at Burlington Primary Care. As a key member of our practice, you will play a vital role in delivering high-quality patient care and supporting our clinicians in providing exceptional healthcare services.

Key Responsibilities
  • Act as the link between clinicians and clinical correspondence, ensuring seamless communication and efficient documentation management.
  • Provide and maintain a high standard of patient communication, adopting a professional and cordial manner at all times.
  • Work alongside clinical and administrative colleagues to facilitate the provision of preventative healthcare to our practice population.
  • Liaise with patients and care facilitate enquiries to deliver healthcare outcomes in line with Practice objectives and contractual responsibilities.
  • Provide secretarial support and cover as needed.
Person Specification
  • GCSE grade A to C in English and Maths.
  • Experience working with members of the public.
  • Experience with using IT.
  • Stellar communication and interpersonal skills.
  • Team player with a dash of independence.
