About the Role

We are seeking a Support Practitioner to join our team to support three male individuals in the southside of Glasgow to live happy independent lives.

The successful candidate will be instrumental in providing assistance within our 24-hour housing support service, catering to individuals with diverse support needs.

Key responsibilities will include:

  • Providing personalized support, carrying out personal care, housework, medication administration, meal preparation, financial management, and fostering companionship.
  • Collaborating with families and professionals to deliver top-quality care tailored to each individual's needs.
  • Advocating for independence and honouring the uniqueness of personal Support Plan.
  • Maintaining records of daily tasks and ensuring they are kept up to date.
  • Working cohesively within a team, fostering effective communication with colleagues, families, and the individuals receiving support.
