We are seeking a highly skilled and experienced Mental Health Practitioner to join our Wisbech Neighbourhood Team, working with older adults in the community. As a key member of our team, you will provide assessment and interventions for individuals with significant mental health conditions, working closely with colleagues to deliver high-quality care.
Key Responsibilities- Conduct comprehensive assessments of mental health needs and develop effective care plans.
- Deliver evidence-based interventions and therapies to support service users in their recovery journey.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and delivery.
- Provide advice and guidance to colleagues on common mental health conditions and best practices.
- Clinical Assessment of mental health needs and development of care plans.
We are looking for a candidate with:
- Relevant professional registered qualification (Registered Mental Health Nurse, Occupational Therapist, or Social Worker).
- Training in supervision and appraisal.
Desirable qualifications include post-qualifying training, non-medical prescribing qualification, and experience of working with older people's community mental health services.
