Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our thriving Primary Care Network in Cambridgeshire. The successful candidate will work independently or as part of the Duty Team to provide high-quality, accessible, and evidence-based care to patients.
About the RoleThe post holder will be responsible for triaging, assessing, planning, and treating emergency or same-day presentations. They will demonstrate safe and effective clinical decision-making skills, including assessment and diagnostic abilities, in face-to-face and telephone appointments.
Key Responsibilities- Assess and identify patients presenting with mental health needs.
- Participate in discussions with the team to improve patient care.
- Clinically examine and assess patient needs from a physiological and psychological perspective.
- Proactively identify, diagnose, and initiate treatment plans for patients at risk of developing a long-term condition.
- Prescribe and review medication in accordance with evidence-based practice and national protocols.
- Use clear referral mechanisms to meet patient needs.
- Prioritize workload and adopt effective time-management strategies.
- Valid and current NMC registration.
- Current membership of a professional organization such as the Royal College of Nursing.
- Enhanced DBS check.
- Full UK driving license.
- Qualified in Phlebotomy.
- Experience of primary care.
