Job Title: Advanced Practice Nurse

Fenland PCN Ltd is seeking an experienced Advanced Practice Nurse to join our team. As an Advanced Practice Nurse, you will be responsible for assessing, diagnosing, and treating patients with complex needs.

Key Responsibilities:

  • Assess and diagnose patients with undifferentiated diagnoses, including those with complex needs.
  • Develop and implement treatment plans, taking into account patient needs and preferences.
  • Prescribe and review medication, ensuring evidence-based practice and adherence to national and practice protocols.
  • Initiate diagnostic strategies and therapeutic interventions, prioritizing patient safety and efficacy.
  • Work collaboratively with patients, GPs, and other healthcare professionals to ensure comprehensive care.
  • Prioritize workload and manage time effectively to meet patient needs.
  • Participate in team discussions to improve patient care and outcomes.

Requirements:

  • Advanced Practice Nurse qualification and registration.
  • Experience in primary care and complex patient management.
  • Strong clinical decision-making skills and ability to work autonomously.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
