Job Summary: We are seeking a skilled and compassionate individual to provide a quality needs-led service for young people preparing to leave care and to provide their support post-care.
Key Responsibilities:
- Provide a leaving care personal advisor role for all eligible, relevant, and former relevant young people as required by the Children (Leaving Care) Act 2000 and volume 3 of the Children Act 2010.
- Produce and review pathway plans and conduct regular 2-monthly visits to care leavers.
- Operate in a mobile flexible service, carrying out duties in normal working hours and evening, and covering working by arrangement.
Requirements: To be successful in this role, you will need to have a strong understanding of the needs of young people leaving care and be able to provide a supportive and non-judgmental service.
