Key Responsibilities
- Provide person-centred care and support to customers, meeting their individual needs and preferences.
- Support customers to develop and maintain links with their local community, promoting social inclusion and reducing isolation.
- Administer medication and provide personal care in line with individual care and support plans.
- Work collaboratively with colleagues and other healthcare professionals to ensure seamless care and support.
