About the Role:
Job Summary: We are seeking a highly skilled and experienced Care Planning Specialist to join our Safeguarding & Quality Assurance Team at Powys County Council.
Key Responsibilities:
- Chairing key care planning meetings to ensure effective decision-making and collaboration among stakeholders.
- Monitoring and reviewing care plans to prevent drift and delay, and to promote the voice of the child and participation and engagement of children and families.
- Advocating on behalf of the child/ren and families, and working effectively with partner agencies to achieve positive outcomes.
- Working effectively within a team and the wider service to ensure seamless delivery of care planning services.
Requirements:
- Excellent chairing skills, including the ability to manage meetings, summarise and present information.
- Minute taking skills for some meetings.
- Good organisation skills, with the ability to work independently and as part of a team.
- Strong networking skills, with the ability to work effectively with partner agencies.
- Ability to keep children and families central to all decision-making.
- Excellent written and oral communication skills.
- Good analytical skills, with the ability to identify and address potential issues.
- Excellent advocacy skills, with the ability to promote the voice of the child and participation and engagement of children and families.
- Promote signs of Safety/ strengths based practice.
