This position offers ongoing opportunities with flexible shifts tailored to the needs of the service and the staff. Candidates must possess substantial experience in caregiving and demonstrate a person-centered approach that is adaptable and dependable. Comprehensive training will be provided, and the induction period is compensated.

The role is centered around being a dedicated, adaptable, and enthusiastic team member within a residential/short break service, committed to delivering a high-quality experience for adults. Collaboration with individuals, their families, and other professionals in a residential setting is essential.

The facilitator will assist adults in line with their care plans, ensuring they have access to social and recreational activities that align with their preferences and requirements. Continuous monitoring of individuals' emotional and overall wellbeing is also a key responsibility.

Key Responsibilities

  • Support individuals in achieving maximum independence and control over their daily lives, as outlined in their care plans.
  • Facilitate access to social and recreational activities, which may include educational classes, community centers, and other engagements.
  • Regularly assess and monitor the emotional and general wellbeing of individuals, safeguarding them from potential harm and reporting any concerns to senior management.
  • Empower individuals to make choices that respect their dignity and privacy, promoting their autonomy in accordance with care plans and relevant documentation.
  • Maintain respectful, patient, and courteous interactions with individuals, their families, visitors, and colleagues, both within the facility and in the community.
  • Ensure accurate and clear documentation that meets Care Quality Commission standards, positively representing Adult Social Care.
  • Establish specific individual and collective goals annually within the performance management framework.

Qualifications and Experience

  • Educated to RQF Level 3 (GCSE) or equivalent through experience, with a commitment to achieving the qualification within 24 months of commencing the role.
  • Demonstrated commitment to ongoing professional development.
  • Experience in a social care environment and effective teamwork.
  • Strong communication skills with the ability to build rapport and engage with diverse individuals.
  • Understanding of a person-centered approach, with respect for dignity and privacy, promoting individual choice.
  • Knowledge of the needs of individuals and relevant legislation concerning Vulnerable Adults, Mental Capacity, and other applicable policies and procedures.
  • Willingness to undergo training in essential areas if not already completed, including Manual Handling, First Aid Awareness, Food Hygiene, Risk and Conflict Management, Data Protection, and Health and Safety.
