About the Role

We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our Acute Medical team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, patient-centered care to individuals presenting with undifferentiated healthcare problems.

Key Responsibilities
  • Assess, investigate, diagnose, treat, and manage patients with complex healthcare needs.
  • Develop and implement effective care plans in collaboration with the multidisciplinary team.
  • Provide leadership and supervision to junior clinicians, promoting a culture of excellence and continuous improvement.
  • Participate in audit, research, and quality improvement initiatives to enhance patient outcomes and service delivery.
  • Stay up-to-date with best clinical practice, adhering to national and local guidelines and formularies.
  • Communicate effectively with patients, families, and healthcare professionals, ensuring seamless care transitions.
Requirements
  • Master's degree in Advanced Clinical Practice or equivalent.
  • Non-Medical Prescriber qualification.
  • Extensive post-registration experience (normally 5 years) in Acute Medicine.
  • Proven leadership and service development skills.
  • Excellent communication, interpersonal, and problem-solving skills.
