Position Overview
Are you a certified nursing professional seeking a role within a community-focused environment? We are pleased to present an opportunity for a part-time Band 5 Nurse to join our inpatient rehabilitation team.
Our facility specializes in rehabilitation and reablement services aimed at facilitating timely hospital discharges, supporting individual recovery journeys, and preventing unnecessary hospital admissions. The patient caseload encompasses a diverse range of medical conditions, including but not limited to orthopaedic, neurological, respiratory issues, and care for the elderly and frail.
Key Responsibilities
The successful candidate will be responsible for addressing the nursing requirements of patients within the inpatient rehabilitation context. You will receive comprehensive support from the Clinical Business Improvement Committee and the Medical Team. In addition to your nursing expertise, you will be expected to conduct general assessments, formulate and execute treatment plans, and actively participate in the multidisciplinary team (MDT).
Your role will include:
- Conducting patient evaluations and setting rehabilitation goals.
- Implementing evidence-based treatment methodologies and management strategies.
- Independently managing your own patient caseload within the rehabilitation framework.
- Facilitating the transition of patients from inpatient rehabilitation to home-based care.
We offer a collaborative work environment alongside Rehabilitation Assistants, Physiotherapists, and Occupational Therapists, all contributing to a comprehensive MDT approach.
The position provides an excellent opportunity for professional growth, allowing you to gain insights and experience while working with a supportive team. You will receive guidance through regular supervision and annual Professional Development Reviews, and you will also play a role in the training and oversight of rehabilitation assistants.
As part of your responsibilities, you will maintain a clinical caseload, ensuring that patients receive high-quality, evidence-based interventions, which you will document in accordance with professional standards and legal obligations.
