Job Summary

Dorset Clinical Commissioning Group is seeking a skilled and dedicated individual to fill the role of District Nurse/ Community Nursing Sister/ Charge Nurse. This position is a key part of our team, working in partnership with The Prince's Trust to support people getting into work and providing employability services to individuals between 16-30 years old.

Key Responsibilities

  • Deliver high-quality healthcare services to patients in the community, working closely with other healthcare professionals to ensure comprehensive care.
  • Provide psycho-educational skills to enhance maintenance of wellbeing using resources provided.
  • Work collaboratively with the team to develop and implement effective care plans, ensuring the best possible outcomes for patients.
  • Conduct risk assessments and provide support to individuals with mental health issues, utilizing knowledge of the Mental Health Sector and the Dorset framework.
  • Develop and maintain strong relationships with patients, families, and other healthcare professionals to ensure seamless care coordination.

Requirements

  • Relevant qualification for the role equivalent to (but not specifically) QCF Level 3 in Health & Social Care.
  • Additional coaching or therapeutic qualification, such as Integrative, Systemic, Person Centred, Psychodynamic, Cognitive Behavioural, and/or Coaching qualifications.
  • Good working knowledge of Microsoft Office 365 applications, email, use of the internet, and social media.
  • Strong interpersonal and communication skills, with the ability to work effectively as part of a team.
  • Ability to adapt to changing situations and priorities, with a flexible approach to work.
