About the Role

We are seeking a skilled and dedicated Community Nurse to join our Redbridge team at NELFT North East London Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality, patient-centered care to our local community.

Key Responsibilities
  • Work collaboratively with a multidisciplinary team to provide holistic care to patients in their own homes.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and support to patients and their families to promote health and well-being.
  • Work closely with local authorities, integrated care networks, and other healthcare providers to ensure seamless care pathways.
  • Travel to various locations within the borough to deliver care, requiring a full UK driving license and access to a car for work purposes.
Requirements
  • Current NMC registration as a Registered Nurse (R/N) or equivalent qualification.
  • Evidence of post-registration education and training.
  • Experience working with professionals from other disciplines and as a member of a team.
  • Ability to work autonomously and provide packages of care involving other agencies.
  • Reflective practitioner with a strong understanding of the NHS Plan, NSF, and clinical governance priorities.
