About the Role

We are seeking a skilled and dedicated Community Nurse to join our Redbridge Community Nursing Team. As a Community Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes.

Key Responsibilities

  • Work collaboratively with a multidisciplinary team to provide holistic care to patients
  • Conduct patient assessments and develop personalized care plans
  • Deliver evidence-based care to patients in their own homes
  • Work closely with local authorities, integrated care networks, and other healthcare providers to facilitate care pathways
  • Travel to various locations within the borough to deliver care

Requirements

  • Current NMC registration as a Registered Nurse
  • Post-registration education and training in community nursing
  • Experience of working with professionals from other disciplines
  • Ability to work autonomously and as part of a team
  • Reflective practitioner with a commitment to ongoing learning and development
