About the Role
We are seeking a highly skilled and dedicated Community Nurse to join our Redbridge Community Nursing Team. As a Community Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes.
Key Responsibilities
- Work collaboratively with a multidisciplinary team to provide holistic care to patients
- Conduct comprehensive assessments and develop individualized care plans
- Deliver evidence-based care to patients with complex needs
- Work closely with local authorities, integrated care networks, and other healthcare providers to facilitate care pathways
- Travel to patients' homes to deliver care, requiring a full UK driving license and access to a car for work purposes
Requirements
- Current NMC registration as a Registered Nurse (R/N) or equivalent
- Evidence of post-registration education and training
- Experience of working with professionals from other disciplines and providing packages of care involving other agencies
- Reflective practitioner with a commitment to ongoing learning and development
