Job Title: Community Mental Health Nurse

We are seeking a skilled and compassionate Community Mental Health Nurse to join our dynamic team at NELFT NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to individuals with complex mental health needs.

About the Role

As a Community Mental Health Nurse, you will be responsible for assessing, planning, and delivering care to individuals with mental health conditions, working closely with multidisciplinary teams and other agencies to ensure comprehensive support.

Key Responsibilities
  • Assess and plan care for individuals with complex mental health needs
  • Deliver high-quality care and support to service users, including medication management and risk assessment
  • Work collaboratively with multidisciplinary teams and other agencies to ensure comprehensive support
  • Provide education and training to service users, families, and carers
  • Contribute to the development and implementation of care plans
Requirements

To be successful in this role, you will need to have:

  • Registration with the Nursing and Midwifery Council (NMC)
  • Experience of working with adults with complex mental health needs
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic awareness of IT and IT skills
