Key Responsibilities

  • Work collaboratively with a multidisciplinary team to provide holistic care to patients
  • Conduct patient assessments and develop personalized care plans

Requirements

  • Current NMC registration as a Registered Nurse
  • Post-registration education and training in community nursing
  • Experience of working with professionals from other disciplines

  • Reflective practitioner with a commitment to ongoing learning and development
