Job Title: Band 6 Primary Care Mental Health Practitioner

We are seeking an experienced Band 6 Mental Health Practitioner to join our team at Sussex Partnership NHS Foundation Trust. As a key member of our community mental health team, you will work closely with GPs and primary care teams to provide high-quality mental health services to individuals with moderate mental health problems.

Job Summary

The successful candidate will work autonomously to a defined caseload of clients, offering specific interventions such as psycho-education, CBT informed sessions, and short-term recovery-focused interventions. You will also have access to regular clinical supervision and support from our Community Leads.

Key Responsibilities
  • Work as part of the secondary care mental health service, acting as a link clinician to primary care teams.
  • Provide support, advice, consultation, and shared care for adults with functional mental illness, enabling safe management of clients within primary care.
  • Work in partnership with GPs and other primary care colleagues to develop care plans for individual patients.
  • Maintain a close working relationship with local secondary care teams, keeping them informed of developments within primary care.
  • Build professional relationships within Primary Care to allow effective development of care pathways within localities.
Person Specification

We are looking for an experienced Mental Health Practitioner with a post-qualifying background in clinical work with people with a range of mental health problems. You should have experience of providing a range of clinical interventions and conducting clinical assessments, including risk assessments.
