We are seeking a dedicated Care Coordinator to join our Complex Care Division at Superior Healthcare. As a Care Coordinator, you will play a vital role in supporting our clients to maintain their health and independence.
Key Responsibilities- Client Management: You will be responsible for managing a caseload of clients, maintaining excellent working relationships with them, and ensuring their needs are met.
- Communication and Event Management: You will work with staff to establish effective employer-employee relationships, minimizing legal risks, and proactively address day-to-day events.
- Quality Assurance: You will play a key role in implementing and maintaining Superior Healthcare's quality assurance programme, ensuring compliance with policies and safeguarding procedures.
- Experience: Previous care coordination experience is essential.
- Interpersonal Skills: Excellent interpersonal skills and a 'team player' attitude are required.
- Autonomy and Teamwork: Ability to work autonomously and as part of a wider team.
- Communication Skills: Excellent oral and written communication skills are necessary.
- IT Skills: Good IT skills, including use of Microsoft Teams & Outlook.
- Competitive Salary: A salary of £25,200, increasing to £28,350 upon successful completion of probation.
